Transcript Request Form

The Transcript Request Form is required for every college you apply to. Once the college application is complete and submitted, you are required to fill out the Transcript Request Form and email a copy to Mrs. O'Brien in the Counseling Office.  This begins the process of Mrs. O'Brien preparing and sending your information out to colleges.

1) Go to Naviance

2) Go to the Document Resources (at the bottom of your home page on Naviance)

3) Go to Senior Information Folder

4) Click on Transcript Request Form

5) Complete for each college to which you have applied

6) Email a copy to Mrs. O'Brien- in the Counseling Office
