Your doctor's office can fill in parts III and IV with date and doctor information from your last physical, or you can email/fax a copy of your last physical (must be within the last year) to (585)637-1853 or arianna.heveron@bcs1.org. BHS students will NOT need to provide a birth certificate, but any students that do not attend BHS will need to provide a birth certificate.
